As an initial matter, Plaintiff did not use the court-approved IFP application form, and the form that she did use does not provide all the information necessary to fully evaluate the request. Additionally, Plaintiff reports that, in the last 12 months, she received $4,019.00 for disability income from the Department of Veterans Affairs. However, based on the information Plaintiff provided and the Court's understanding of VA disability income, the Court presumes that Plaintiff actually receives this amount each month. Supp. 3d 715, 717 (N.D. Ill. 2021). To proceed with this case, by 2/6/25, Plaintiff must either (1) pay the filing fee in full; or (2) file a new in forma pauperis application that accurately discloses her financial condition using the court-approved form, which is available at https://www.ilnd.uscourts.gov/_assets/_documents/_forms/_online/IFP_nonPrisoner.pdf. Failure to do so will result in the case being dismissed without prejudice.
